Terms of Service

Effective Date: April 9, 2026

Website Terms of Service

Welcome to the Polaris Psychology website. These Terms and Conditions govern only your use of the public Polaris Psychology website, public website forms, and non-clinical communications relating to general inquiries, scheduling, website use, and related administrative matters.

By accessing or using our Site, you agree to be bound by these Terms. If you do not agree with any part of these terms, you must not use our Site.

Scope of These Terms

These Terms and Conditions govern only your use of the public Polaris Psychology website, public website forms, and non-clinical SMS communications relating to general inquiries, scheduling, and billing.

These Terms and Conditions do not govern the provision of psychological, healthcare, or other professional services. Any therapy, psychological evaluation, psychological assessment, telehealth services, billing obligations, informed consent, privacy rights relating to protected health information, or other clinical or professional services are governed by separate intake, consent, privacy, practice policy, and client service documents provided by Polaris Psychology.

This Website Is Informational Only

The content on this Site, including articles, “resource” posts, educational materials, and service descriptions, is provided for general informational purposes only.

  • It is not medical, psychological, mental health, or legal advice.
  • It does not create a psychologist-client, therapist-client, or other professional relationship.
  • Submitting a website form, sending a text message, calling us, requesting information, or scheduling a consultation does not by itself establish a professional relationship with Polaris Psychology.
  • A professional relationship is formed only if Polaris Psychology determines that we are able to provide services and the appropriate intake, consent, and practice documents have been completed.
  • Submitting a form, sending a text, calling us, or scheduling a consultation does not guarantee acceptance as a client, scheduling availability, or the provision of services.
  • Any actual clinical or professional services are governed by separate intake, consent, privacy, practice policy, billing, telehealth, and client service documents provided by Polaris Psychology. Telehealth and telepsychology services may also be subject to additional jurisdiction-specific requirements, informed consent, and emergency planning procedures.

Access to this website does not mean that clinical services are available in every state or territory. Clinical services, including telehealth services, are offered only where Polaris Psychology and the specific clinician involved are legally authorized to provide them. Availability depends on the client’s physical location at the time of service, the clinician’s licensure or other authority to practice, and applicable law. Do not assume that submitting a form, requesting a consultation, or viewing service information means that services are available in your location.

Do not rely on information from this Site as a substitute for professional advice, diagnosis, treatment, or legal guidance. If you are in immediate danger or experiencing an emergency, call 911. If you need mental health crisis support, call or text 988 or go to your nearest emergency department.

Appointment Locations; Mailing Address

Some locations listed on this website are appointment-only service locations. Unless Polaris Psychology expressly states otherwise, those locations are not continuously staffed administrative offices, general mailing addresses, or locations designated to accept routine mail, legal notices, subpoenas, or service of process.

Official correspondence should be directed to Polaris Psychology’s designated mailing address or, where applicable, its registered agent or other address reflected in applicable public filings.

Nothing in this section is intended to limit, waive, or alter any method of service, notice, or legal process permitted or required by applicable law.

Official correspondence should be directed to:

52 Nashua Street
Suite 19
Milford, NH 03055

Do Not Send Urgent or Sensitive Information Through This Site or SMS

Please do not use this Site, its public forms, or ordinary text messaging to send urgent, emergency, highly sensitive, or confidential clinical information. These channels may not be monitored at all times and may not be appropriate for time-sensitive communications.

Submitting a website form or sending a text message does not create a psychologist-client relationship, does not guarantee a response by any particular time, and is not an appropriate way to seek crisis support or emergency care.

If you are in immediate danger or experiencing an emergency, call 911. If you need mental health crisis support, call or text 988 or go to your nearest emergency department.

Use of the Site

You agree to use this Site only for lawful purposes. You may not use the Site in any way that could damage, disable, or impair the Site or interfere with any other party’s use of it.

Intellectual Property

Unless otherwise stated, the content on this Site, including Polaris Psychology’s text, graphics, design elements, original images, and other original materials, is owned by Polaris Psychology or used by Polaris Psychology with permission or other lawful authority, and is protected by applicable United States and international intellectual property laws. Certain trademarks, service marks, trade names, logos, seals, images, and other materials displayed on this Site may be owned by third parties and remain the property of their respective owners. You may not reproduce, distribute, modify, display, or create derivative works from Site content except as permitted by law or with the applicable owner’s permission.

Third-Party Trademarks and Affiliations

Any and all third-party trademarks, service marks, trade names, logos, and seals (including but not limited to the American Psychological Association, National Register of Health Service Psychologists, PSYPACT, WebMD, Vitals, ChoosingTherapy.com, Psychology Today, Therapy Den, ASPPB, ABPP, University of New Hampshire, and Antioch University) displayed on this website are the registered or unregistered intellectual property of their respective owners.

Polaris Psychology displays these third-party marks only to accurately describe factual professional affiliations, memberships, directory listings, credentials, educational backgrounds, or other relevant relationships, and does so only as permitted by applicable law or other applicable authorization. Polaris Psychology is an independent private practice. The display of these marks does not imply that Polaris Psychology is affiliated with, an agent of, or endorsed or sponsored by these respective organizations beyond the factual, stated relationship (e.g., active membership, verified profile listing, alumni status, etc.).

Third-Party Services and Links

Our Site may contain links to third-party websites, platforms, or services that are not owned or controlled by Polaris Psychology.

We are not responsible for the content, availability, privacy practices, security, or terms of any third-party websites or services. Your use of any third-party website or service is governed by that third party’s own terms, conditions, and policies.

These Terms do not govern any client portal, telehealth platform, scheduling platform, payment processor, phone or messaging platform, electronic health record system, or other third-party technology used in connection with actual or prospective professional services.

Disclaimer of Warranties

This Site is provided on an “as is” and “as available” basis. Polaris Psychology makes no representations or warranties of any kind, express or implied, as to the operation of the Site or the information, content, or materials included on it. We do not warrant that the Site will be uninterrupted, error-free, or free of viruses.

Limitation of Liability

To the fullest extent permitted by law, Polaris Psychology, its clinicians, and its affiliates will not be liable for any indirect, incidental, consequential, special, exemplary, or punitive damages arising out of or relating to your use of, or inability to use, the public website, public website forms, non-clinical SMS communications, or any informational content made available on the website.

This limitation applies only to claims arising from use of the website, public forms, or non-clinical SMS communications. It does not alter any rights or obligations that may arise under separate client service, informed consent, privacy, billing, telehealth, or other professional agreements.

Your sole remedy for dissatisfaction with the Site is to stop using it.

Governing Law and Venue

These Terms and any dispute arising out of or relating to these Terms or your use of the public website, public website forms, or non-clinical SMS communications shall be governed by and construed in accordance with the laws of the State of New Hampshire, without regard to its conflict of law principles, except to the extent applicable non-waivable law requires otherwise.

Any legal action or proceeding arising out of or relating to these Terms or your use of the website, public forms, or non-clinical SMS communications shall be brought in the state or federal courts located in New Hampshire, except where another forum is required by applicable law.

Severability

If any provision of these Terms is found to be invalid, unlawful, or unenforceable, that provision shall be enforced to the maximum extent permitted by law, and the remaining provisions of these Terms shall remain in full force and effect.

Waiver

No waiver by Polaris Psychology of any provision of these Terms shall be deemed a further or continuing waiver of that provision or any other provision. Our failure to enforce any right or provision under these Terms shall not constitute a waiver of that right or provision.

Entire Agreement

These Terms constitute the entire agreement between you and Polaris Psychology with respect to your use of the public website, public website forms, and non-clinical SMS communications, and they supersede any prior or contemporaneous understandings, communications, or agreements relating to that subject matter.

These Terms do not govern the provision of psychological, healthcare, or other professional services, which are governed by separate intake, consent, privacy, billing, practice policy, telehealth, and client service documents provided by Polaris Psychology.

Changes to These Terms

We reserve the right to modify these Terms at any time. We will post the most current version on our Site. Your continued use of the Site after any changes indicates your acceptance of the new Terms.
